It could be real. Most movie and TV casts goof around a little bit on-set. I'm not photoshop expert,though, so I can't offer a very informed opinion.
One thing I do notice, though, is that Chewie's fingers are clearly visible, whereas they're usually hidden in his fur.

I say it's fake. There's something weird about Leia's face... her neck looks too skinny (compared to her body) and her face looks too long. Just look at her open mouth and how it makes her lower jaw extremely long.
And btw, her face looks DAMN CREEPY in that pic.
It could be real... maybe it's a very good Carrie Fisher lookalike posing with a guy in a Chewie outfit at a comic convention or something?

I think it is from a goof-off moment on the set. I remember years ago when someone had collected a large amount of goof-off moments from the show Dallas, and started to market them as a video tape. The initial reaction was, "JR would never do that, and what exactly are Bobby and Pamela doing under that blanket??"
My vote is on true, but goofy.

The general position of Chewbacca and Leia at least make a snapshot as improbable as it gets. This must be either photoshopped (my guess) or at least staged, in which case the "never meant to be seen" tag gets suspicious.

According to IMDB.com- Peter Mayhew (Chewbacca)= 7' 3" (2.21 m) tall Carrie Fisher (Leia)= 5' 1" (1.55 m)
So I have to agree, with Plunkettios, if it's real, one of them is not the "real" character, and since Chewy wears a giant hairy suit, he would be easier to replace with a stand-in.
ETA- I do find it amusing that you can see either air or eye holes in Chewie's tongue

I'm not sure why everyone is bringing up the height difference. The picture only shows them from the midsection up. It would a simple thing to crouch down, and a logical thing to do if you were trying to take this picture. It looks posed to me, and I'm sure you'd want to get their faces close to the same level.

Hey, I just watched ESB a few weeks ago, and I can't off the top of my head think of any scenes from the Hoth sequence that contained a background set like the one in the picture. I could be wrong, but otherwise, I think that would be a reason to vote either fake or some seperate shoot, like for a magazine.

Like calle and Tootsie have said, I vote for magazine shot/gag shot. Just because the quality of the picture, would assume to me it was taken with a professional camera.
Although my first reaction was it might have been taken as a gag photo on the set of SNL or the likes. Does any know if she hosted about that time?

In the ESB photo gallery on the Return of the Jedi DVD, there is a never-seen-before publicity photo of Leia in that exact same outfit standing in front of a similar background so I believe this photo could most definitely be a real photo that was taken as a gag and probably never meant to be seen.

There seem to be a few photos of her in that same outfit with the exact same hairdo too, plus the DVD evidence mentioned. At first I thought Leia may be a lookalike but you can see a unique characteristic of her eyes. Her right eye looks slightly to the right in many photos of her. I also find her expression "convincing" as someone who is in on the joke, having some fun on the publicity shoot. I'm leaning heavily towards "real" on this.
