Being the good Canadian that I am, I play the hockey video games from time-to-time.
After having read the UL about the Madden game jinx, I am reminded of the fates of some of those who graced the EA's NHL series of games.
Marcus Nasland and Jarome Iginla both had horrible seasons after being on the cover. Danny Heatly crashed his car (injuring himself and killing a teammate [I can't recall the name of this teammate right now... Schiender or something like that maybe?])
He can't think of any others right now... Vincent Lecavalier didn't have too bad of a season after he was on the cover. Can anyone else think of any others who had misfortunes?

There was a thread in here somewhere about all the EA Games having a jinx attached to it.
The baseball one was pretty accurate up until recently when Manny Ramirez was put on the cover of the 2005 version.

ManRam had a great season in 2005, but he was hurt and his BA and OPS were his lowest since 1998.

There have been several players on the cover of NCAA Football that busted out of the pros, but that's fairly common so I'll only mention one of significance. Reggie Bush is on NCAA 2007, and now allegations have surfaced that might cause him to lose his Heisman Trophy and National Championships. That's pretty damned bad.
