The one with the cop holding the gun without his finger on the trigger might not qualify as a mistake even if its legit. Some police and security forces actually teach you not to put your finger on your trigger until your ready to fire. It's not the preferred method usually, but it is used in some places.

You wanna see real ones, watch Cave Dwellers, a Conan the Barbarian knock-off that was featured in MST3K. Crow points out a few of them in a segment at the end: tire tracks, modern houses, and even a guy wearing sunglasses.
