A while back, my friend Heather bought a copy of Nirvana's Nevermind CD. It was shrinkwrapped, and had all the seals still on it and all that sort of thing. Eventually, she popped out the CD tray (don't remember why, might've broken the jewel case) When she opened it, she found a note written on the other side of the CD's tray liner.
Her husband, my friend Steve, sent me a scan of it. I posted it here:

I can't find a picture of Kurt's autograph to compare and contrast, but "bored factory worker" would be my guess, too.
My mate and I did something vaguely similar a while back. We hired out "Tony Hawks 3" for the PS2 and got quite far before resorting to phoning Jake's brother and asking for the cheat codes. When it came time to return the game, we returned it with a hidden note detailing all the cheat codes for the next person.
We also once hired out a video that had the legend "this film is shit" written on the side. It might have been Cherry Falls.

This is a very cool story, if it's not true it should be dangit!
But if it was bought new, wouldn't that copy have to have spent much of the '90s and early into the '00s sitting in a warehouse before being put on a store rack just recently. I would think shipments would sell out in less time than that, especially if you consider this would be the sort of thing to be put in a first pressing and "Nevermind" wasn't really expected to be as much of a smash hit as it turned out to be (less copies printed up, so the initial ones sold out fast).
